Welcome to 701 Chiropractic & Rehab

Visiting a chiropractor for the first time can feel a little like stepping into uncharted territory, perhaps you’re not entirely sure what to expect. At 701 Chiropractic & Rehab, we’re here to make this process as seamless as possible, focusing on what matters most: getting you back to feeling your best, and fast.

To get started, we’ll need to gather some essential information—think of it as the foundation of your treatment plan. This includes your demographics, health history, and a few outcome assessments to guide us. We ask that you complete this paperwork before your scheduled appointment. If life gets in the way and you’re unable to do so, no problem, just arrive 20 minutes early to fill it out, and we’ll take it from there.

✅ Before Your Appointment: A Few Helpful Notes

To make sure your first visit goes as smoothly as possible, here are a few things to keep in mind:

📄 Complete your paperwork ahead of time.

Please fill out your new patient forms online before your scheduled appointment. If you’re unable to do so, plan to arrive at least 20 minutes early to complete them in the office.

👕 Dress comfortably.

We recommend wearing athletic, loose-fitting, or comfortable clothing that allows you to move freely during your exam and treatment.

💳 Don’t forget your insurance card.

If you’re using health insurance, be sure to bring your current insurance card with you to your first visit.

🚗 Involved in a car accident or work injury?

Let us know ahead of time if your visit is related to a motor vehicle accident or a workers’ compensation case. This helps us make sure the right paperwork and billing steps are in place.

Your first appointment will typically take between 45 minutes to 1 hour. During this time, you will sit down to discuss the reason for your visit and your goals for care. This will be followed by a musculoskeletal exam where we will assess your posture, range of motion, and movement strategies. We will also perform special tests to help determine which tissues could be contributing to your pain. If appropriate, this will be followed by your first treatment.

Think of it like a roadmap, we’re not just looking at where it hurts but figuring out why it hurts, how it started, and what we can do to help you confidently move forward. Our goal is to give you answers, a clear plan, and most importantly, some relief.
